To facilitate the paperwork necessary to initiate physical therapy, please fill out the Patient Intake Form, Authorization to Release Medical Information & please fill out the Outcome Measure that matches the body part we will be treating. Please feel free to call our office with any questions you may have regarding physical therapy.
